Registration for Vendors is now CLOSED. 

There is NO CHARGE to participate in the Disability Ministry Resource Expo. We only ask that you DONATE A DOOR PRIZE ITEM valued at no less than $20. This can be a gift basket, gift cards, items from your agency, etc. Be as creative as you like with the door prizes. Gift donations will be raffled to our guests throughout the “Spring Butterflies” themed event.

Please prepare to have an interactive activity, giveaways and/or treats for the expo visitors, such as a spin the wheel, bag toss, candy and/or goodies, etc. We want visitors to get the real experience of your agency and what you have to offer their loved ones.

Registration will begin at 9:00am. You will register, drop off your door prize item and will be guided to your set up table. A 6ft. table and 2 chairs will be provided for your space. If you require outlets, or any other items please let us know on this form so that we may try and meet your needs.

By signing below, you agree that you will participate in the 2019 Dallas Disability Ministry Resource Expo at the date and time noted above OR will notify the Disability Ministry committee as soon as possible, in the event of a needed cancellation.
